3. Worn rollers that jumped the rail
Rollers that wobble on worn stems eventually track badly enough to leave the rail on their own — often on a curve, where the track's grip on the roller is weakest.

Tracks that were already damaged, loose on their brackets, or spread apart give the rollers room to escape. Sometimes the bent track is the original cause; sometimes it's damage from a previous partial derailment that was never addressed.
